Patient information & consent

Medical consultation (on site or via video), data processing, prescription delivery & delivery

By completing the registration and booking an appointment, I confirm the following points and — if necessary — give my consent. I can withdraw my consent at any time with effect for the future.

1) Medical consultation (on site or via video)

My medical consultation can — depending on availability — be provided in person or via secure video consultation. The medical evaluation and treatment decision are made exclusively in consultation with the doctor and are duly documented.

Note in case of emergencies: Advice on enmedify does not replace emergency care. In acute emergencies, I contact the emergency service (144), my family doctor or the nearest hospital.

2) Data protection & communication

I have been informed about the processing of my personal and medical data in accordance with DSG/GDPR. Insofar as consent is required for this, I give this expressly for the purposes of my treatment with enmedify. I agree that enmedify may contact me for treatment purposes (e.g. appointment reminders, inquiries) via the contact channels I have provided (e-mail, telephone, SMS). Withdrawal is possible at any time.

3) Prescription transmission to partner pharmacy & waiver of free choice of pharmacy

I agree that medical prescriptions will only be sent to a partner pharmacy named by enmedify, approved in Switzerland and approved for delivery.

I expressly refrain from freely choosing the dispensing pharmacy for this treatment; delivery is provided exclusively by this partner pharmacy. Withdrawal is possible at any time with effect for the future; supply via the EnMedify shipping model is then no longer possible.

4) Dispatch of medicines

Delivery is only made on the basis of a valid medical prescription recognized in Switzerland and exclusively to delivery addresses in Switzerland. The partner pharmacy carries out appropriate identity and address checks and delivers with proof of delivery (e.g. against signature; “personally”, depending on the product).

This consent to dispatch is considered a standing consent and is documented in my file for each order; it is not necessary to obtain it again per order. Withdrawal is possible at any time with effect for the future.

5) Form of prescription

Prescriptions are submitted and processed in the form permitted by law (including special formal requirements, such as narcotic prescriptions, where necessary). Pure copies or unsigned documents cannot be accepted as a valid prescription.

6) Safe handling of medicines (including cannabis)

I use prescribed medicines as intended and store them in such a way that they cannot be accessed by third parties. Disclosure to third parties is prohibited and may have legal consequences.

If there are side effects or problems, I will inform enmedify or the partner pharmacy; I will immediately report emergencies to the emergency service.

7) Pregnancy/breast-feeding and medication list

I have provided information on pregnancy/breast-feeding as well as on all medicines, allergies and intolerances currently being taken and will immediately communicate any changes, as these are essential for a safe treatment decision.

8) Cost notice

I am aware that consultations and services are billed in accordance with the applicable rates and may incur costs for me depending on the insurance/coverage.

9) Validity & Withdrawal

This document is valid until I withdraw it. A revocation does not affect the lawfulness of the processing or delivery carried out up to that point. Individual points (e.g. shipping, prescription transfer to the partner pharmacy) can be withdrawn; supply via the respective model may end as a result.
