Patient Information & Consent

Medical consultation (on-site or via video), data processing, prescription delivery & shipment

By completing my registration and booking an appointment, I confirm the following points and — if necessary — give my consent. I may withdraw my consent at any time with effect for the future.

1) Medical consultation (on site or via video)

My medical consultation will be conducted — depending on availability — either in person or via secure video consultation. The medical assessment and treatment decision are made exclusively in consultation with my doctor and are duly documented.

Note on emergencies: enmedify health does not provide emergency care. In acute emergencies, I will contact the emergency services (144), my family doctor, or the nearest hospital.

2) Data Protection & Communication

I have been informed about the processing of my personal and medical data in accordance with the Swiss Data Protection Act (DSG) and, where applicable, the EU General Data Protection Regulation (GDPR).

To enable medical supervision and coordination of my treatment with enmedify health, I agree that enmedify health may contact me for treatment-related purposes (e.g., reminders, updates) via the contact channels I have provided (e-mail, telephone, SMS).
Consent may be withdrawn at any time.

3) Prescription Transmission to Partner Pharmacy & Waiver of Free Choice of Pharmacy

Prescriptions for my medical treatment may be sent only to a partner pharmacy named by enmedify health, unless I expressly request otherwise.

If I choose a different pharmacy, I release enmedify health from its obligation to coordinate the dispensing process for this treatment. Delivery via the enmedify health shipping model will then no longer be possible..

4) Dispatch of Medicines

Delivery is made only on the basis of a valid medical prescription recognized in Switzerland and exclusively to addresses within Switzerland.

The partner pharmacy carries out appropriate identity and delivery checks and delivers with proof of delivery (e.g., signature, “personal” delivery, depending on the product).
Delivery is documented in my file for each shipment. Renewal of consent is not required for each individual delivery. Consent may be withdrawn at any time with effect for the future.

5) Form of Prescription

Prescriptions are submitted and processed in the legally permitted form (including digital formats, where applicable). Pure copies or unsigned documents cannot be accepted as valid prescriptions.

6) Safe Handling of Medicines (Including Cannabis)

I use prescribed medicines responsibly and store them securely to prevent access by unauthorized persons. Redistribution or misuse of the medicine is prohibited and may result in legal consequences.

If any adverse effects or problems occur, I will immediately inform enmedify health, my treating physician, or the partner pharmacy.

7) Pregnancy/Breastfeeding and Medication List

I have provided information regarding pregnancy and/or breastfeeding, as well as on all medicines, allergies, and dietary supplements I am currently taking. I will immediately communicate any relevant changes, as these factors are essential for safe treatment decisions.

8) Cost Notice

I am aware that all consultations and services are billed in accordance with applicable rates and may incur additional costs depending on my insurance coverage.

9) Validity & Withdrawal

This document remains valid until it is withdrawn. Withdrawal of consent does not affect the lawfulness of prior data processing or actions carried out up to that point (e.g., shipping, prescription transmission to the partner pharmacy).After withdrawal, delivery through the enmedify health partner pharmacy may no longer be possible.
